They seek to improve the proportions of the nipple areola complex in relation to the mammary gland.

The areola-nipple complex is located at the point of maximum projection of the breast and its morphology and location can alter the appearance of the breast. For this reason it has a great aesthetic and psychological importance.

The ideal dimensions of the areola are about 4 cm and the nipple 1 cm in diameter by 1.2 cm of projection.

There are several anatomical variables related to the areola-nipple complex (CAP)

polythelia : there are more CAPs than usual (which is one per breast). In this congenital calteration of the breast, supernumerary nipples are seen, usually in the submammary grooves, in both girls and boys. It may be related to nephrourological malformations so they should be ruled out.

Atelia : it is the absence of CAP and may be related to other malformations such as Poland syndrome.

Macrothelia : it is characterized by a large CAP with respect to the breast. Usually it is secondary to pregnancy or lactation or typically in tuberous breasts

Microtelia : it is characterized by a small nipple. It usually occurs in malformations such as Poland syndrome or congenital breast hypoplasia.

Excess projection of the nipple: the maximum projection of a nipple has been established at 1.2cm. Projections higher than this figure may be reduced.

Excess diameter: The standard diameter of a nipple has been established at 1 cm. Disproportionate diameters can be surgically corrected.

Inverted or retracted nipple: l benign invagination of the nipple appears in up to 4% of women. It can be caused by pregnancy, breast surgery, mastitis, or breast cancer.

Anisothelia : it is the asymmetry between the CAPs of the same patient. It is usually a frequent malformation.

– Depending on the situation of the nipple, it can be supraaerolar or exoareolar, rare malfunctions.

Preoperative

In order for you to be able to undergo an areola-nipple complex surgery in complete safety, we must assess your general condition based on your possible drug and environmental allergies, personal and family pathological history, habitual and past medication, possible previous surgical interventions, toxic habits (tobacco) , etc.

It is also necessary to have a general analysis, electrocardiogram and sometimes a mammogram.

Surgical technique

We perform the surgery in the operating room, under local anesthesia.

The areola-nipple complex intervention usually lasts between 30 minutes and 1 hour.

We perform different techniques depending on the malformation to be corrected.

– Macrothelia: correction usually requires a periareolar scar.

– The excess of projection is reduced by eliminating a skin cylinder at the base of the same and keeping intact the erector muscles and the plexuses that innervate and vascularize the tip of the same. The scar will be imperceptible.

– Excess diameter: to correct this diameter, a resection of one or more nipple wedges is performed.

– The symmetry between the two areola-nipple complexes require different techniques depending on the malformation.

At the end of the surgery a bandage is placed.

 

POSTOPERATIVE

This surgery is outpatient.

It is normal for inflammation, pain sensation, occasional stinging and tingling or burning in the nipples to appear, which you can calm with medication and postoperative treatment as well as cryotherapy.

Between the fourth or sixth day, we do the control in the consultation where we will remove the bandage and begin neural therapy for your greater comfort. We recommend the use of a special seamless sports bra, which you must wear continuously day and night for four weeks.

You will be able to incorporate relatively quickly into daily and work life, always following the specialist’s instructions, limiting strong physical activities for a few weeks, until you recover from the intervention.

The sutures are removed between 7 – 10 days after the operation and the aesthetic treatment of the scars begins so that in the future they are as imperceptible as possible. Scars appear pink at first. From the 2nd or 3rd month they improve aesthetically. The final appearance of these scars is variable, depending on the characteristics of the skin of each person.

Can I lose feeling?

The sensitivity of the chest and more specifically of the area of the areola and the nipple, depends on sensitive nerves called intercostals, which, as their name suggests, cross the intercostal muscles, reaching the surface of the skin sensibly innervating the areola. During the procedure, these nerves can be damaged. Sensitivity alterations usually recover over time.
